Efcacy of Enhanced External Counterpulsaton (EECP) in Nepalese chronic stable Angina patent: a single centre prospectve study at Shahid Gangalal Natonal Heart Center (SGNHC)

Abstract

Background Enhanced External Counterpulsaton (EECP) is a novel; FDA approved; non-invasive; outpatent treatment offered to patents with refractory angina pectoris. It uses sequentally inflated pneumatc cuffs on the lower extremites to enhance coronary diastolic flow. We studied its effect in twenty one patents with refractory angina pectoris. Methods All patents (n=21) who were referred for Enhanced External Counterpulsaton to Shahid Gangalal Natonal Heart Center Enhanced External Counterpulsaton Llb who completed a treatment course (one hour per day for 35 days) of EECP and underwent 6-minute walk test before and afer treatment were included. Demographic data, coronary artery disease risk factors and baseline angiographic data were collected. Distance covered in six minute walk test before and afer the treatment was compared. Results All the patents who had undergone Enhanced External Counterpulsaton had a positve clinical response. Distance covered in six minute walk test improved in all patents afer the treatment. Decrease in anginal severity, frequency and the use of sublingual nitrates, with improvement in quality of life was observed afer the treatment. During the treatment some patents complained of leg pain, one patent developed blister and one ecchymosis but the treatment was not discontnued. Conclusion The results from this study suggest that Enhanced External Counterpulsaton is an effectve, safe and well tolerated treatment opton for the patents with refractory angina pectoris.
