Effect of pain intensity on psychological state of patients with epidermolysis bullosa

Abstract

The term «congenital epidermolysis bullosa» (EB) unites a group of rather rare, genetically and clinically heterogeneous diseases characterized by the formation of a certain amount of blisters and erosions (from single to multiple) in various parts of the skin and mucous membranes in the utero or in early childhood. Clinical manifestations of EB have a significant negative impact on the quality of life of patients and their families, not only in the physical, but also in the psychological aspect. In patients with a severe form of the disease, blisters and wounds constantly appear, accompanied by pain and itching. Lesion covers a large area, complications develop that lead to disability. The severe course of the disease can cause neurosis, which leads to social maladaptation of patients, complicates the processes of learning, work and rest. Objective — to estimate the effect of pain in patients with epidermolysis bullosa on their quality of life with a statistically significant sample from the general Ukrainian patients population using BIS, HADS, DN4, ВАШ scales. Materials and methods. Materials and methods. To assess the effect of pain on the psychological state of patients with EB and measure the intensity of pain, the HADS, VAS scales, the DN4 questionnaire, and others were used. Results and discussion. We recorded an extremely significant and clearly negative impact, indicated by patients during their examination, of somatic lesions of both skin and other organs on the condition of quality of life. One of these negative factors, daily affecting quality of life, is the pain in patients. We have clarified the content component of pain which has the following characteristics: burning (in 15 patients), or excruciating cold (in 6 patients), or a sensation like an electric shock (in 8 patients). It was found that the strength of one or another above types of pain in the majority of patients (13 of 19) was assessed as «significant». Assessment of anxiety­depressive disorders by HADS showed that they were present only in every 5th – 6th patient. Conclusions. For the first time, a scientifically based combined study was conducted to assess the effect of persistent pain during EB on the quality of life of such patients with a statistically significant sample of the total Ukrainian patient population.
