Effects of robot-assisted upper limb training combined with repetitive transcranial magnetic stimulation on improving upper limb function in stroke patients

Abstract

Objective To investigate the effects of robot-assisted upper limb training combined with repetitive transcranial magnetic stimulation (rTMS) on improving upper limb function, upper limb spasticity and activity of daily living (ADL) in stroke patients. Methods Seventy stroke patients who met the inclusion criteria and were hospitalized in Xijing Hospital, Air Force Medical University from January 2020 to December 2021 were randomly divided into the control group (n = 35) and the robot group (n = 35). The control group received conventional occupational therapy combined with rTMS (hand representation of primary motor cortex of unaffected side, 1 200 pulses, 80% resting motor threshold), and the robot group received robot-assisted upper limb training combined with rTMS (with the same parameters as the control group). The robot-assisted upper limb training was performed once a day, 20 min each time, 5 times a week for 4 consecutive weeks. Both groups received the same conventional rehabilitation treatment. The Wolf Motor Function Test (WMFT), Brunnstrom stage, Fugl-Meyer Assessment of Upper Extremity ( FMA-UE), Modified Ashworth Scale ( MAS) and Modified Barthel Index (MBI) were used for evaluation. Results Before treatment, WMFT, Brunnstrom stage of upper limb and hand,FMA-UE, MAS and MBI of upper limb were used in the two groups, and there was no significant difference between the two groups (P > 0. 05). After 4 weeks of treatment, the two groups of patients were evaluated by the same method. There was no significant difference in MAS of the upper limb compared with that before treatment (P > 0. 05), and all the others were significantly improved compared with those before treatment ( P < 0. 01 ). WMFT, Brunnstrom stage of upper limb, FMA-UE and MBI were used in the robot group, and the improvement was more obvious than that in the control group after treatment (P < 0. 05 or P < 0. 01), but there was no significant difference in Brunnstrom stage of hand and MAS of upper limb between the two groups after treatment ( P > 0. 05). Conclusion Systematic rehabilitation more than 4 weeks can effectively improve upper limb function and ADL of stroke patients, and robot-assisted upper limb training combined with rTMS can better improve upper limb motor function of stroke patients.
