Endoscopic treatment of sphenoidal meningoencephalocele with accompanying nasal rhinorrhea – case report

Abstract

BACKGROUND Meningoencephalocele is a rare pathology, characterized by protrusion of the meninges and/or cerebral structures, due to bone defect in the skull or cranial base. It is manifested relatively more frequently in children, already in their first months of life. In adults, it is usually diagnosed accidentally, in the course of diagnostics of spontaneous nasal rhinorrhea or recurring cerebrospinal meningitis. MATERIAL AND METHODS This study reports two cases of patients who underwent surgical treatment due to nasal rhinorrhea, secondary to meningoencephalocele. In the first patient, spontaneous rhinorrhea was the only manifestation of the meningoencephalocele, in the second the signs were nasal rhinorrhea and a history of bacterial cerebrospinal meningitis. In both patients, an endoscopic endonasal reconstructive operation of the bone defect was performed, with reduction of the hernia, in one case an external lumber drainage was additionally applied. CONCLUSION Each case of spontaneous nasal rhinorrhea calls for thorough analysis, with particular emphasis on medical imaging. Despite the rare occurrence of meningoencephalocele in adults, one should keep it in mind as a cause of nasal rhinorrhea, because correct diagnosis and treatment of this pathological condition creates the possibility of curing the patient completely, and prevents dangerous complications.
