European Position Paper on Rhinosinusitis and Nasal Polyps 2012 – most current knowledge on rhinosinusitis

Abstract

The European Position Paper on Rhinosinusitis and Nasal Polyps 2012 (EPOS 2012) is the updated version of EPOS document, published in 2005 and 2007. Among the chairs and authors of EPOS 2012, similarly like previously are: W.J. Fookens,V.J. Lund, J. Mullol and C. Bachert. From Poland – prof. Marek Kowalski from Lodz participated in preparing this document. It contains chapters on definitions and classification, with new term of difficult to treat rhinosinusitis and control of disease, as well as novelized, better description ofrhinosinusitis in children. In fact, the chapters on paediatric acute and chronic rhinosinusitis are totally rewritten comparing to previous issues.The authors put more attention on the diagnosis and treatment of acute rhinosinusitis. Although still the chronic rhinosinusitis is divided into: CRS with nasal polyps and CRS without nasal polyps, but the differences between these two entities are described, especially in pathophysiology and treatment.There are some new data on epidemiolgy, coming from latest studies, predisposing factors, inflammatory mechanisms, differential diagnosis of facial pain, genetics, cystic fibrosis, aspirin exacerbated respiratory disease (AERD), immunodeficiencies, allergicfungal rhinosinusitis (AFRS) and the relationship between upper and lower airways. EPOS 2012 is addressed not only to ENT-specialists but also to primary care and non-ENT specialists.
