Evaluation Of Clinical Results And Complications Of Internal Fixation Of Proximal Humeral Fractures With The Proximal Humerus Locking Plate

Abstract
Introduction:A proximal humeral fracture is a common injury to the shoulder and account for approximately 5% of all fractures and 26% of all humeral fractures. Despite recent advancement in management of proximal humeral fracture, the outcomes are often poor. The aim of the present study was to evaluate the clinical results and complications of internal fixation of proximal humeral fractures with the proximal humerus locking plate. Material and Methods:The present study included 21 patients (9 males; 12 females, mean age 53.8 years; range, 32 to 68 years) of proximal humeral fractures who were treated with open reduction and internal fixation with proximal humerus locking plate between March 2010 to May 2014. Out of 21 patients, 3 patients (14.2%) had two part fracture, 7 patients (33.3%) had three part fracture, 9 patients (42.8%) had four part fracture and 2 patient (9.5%) had four-part fracture dislocation. Standard deltopectoral approach was used in all patients. Bone grafting was not done in any of the patients. In all patients similar standard physical rehabilitation therapy were followed. All complications including intra and postoperative were assessed and recorded. The patients were followed up for over 24 months. Functional outcome was assessed using the constant shoulder score. Results:In the present study 18 (88%) patients achieved union in good anatomical position. In 3 patients, fracture union was achieved in varus position. Complications were seen in 5 (23%) patients. Complications encountered were intraarticular screw penetration in 4 (19%) patients, avascular necrosis of humeral in 2 (10%) patients, subacromial impingement in 2 (10%) patients and infection was found in 1(5%) patient. Functional result were excellent in 6 (29%) patients, good in 9 (43%), fair in 4 (19%) and poor in 2 (9%) patients. Mean CMS score was 72.38. However, the mean constant score showed significant differences among different type of proximal humeral fracture. Conclusion:Proximal humerus locking plate represents a significant treatment option in management of proximal humeral fractures with low complication rate.
