Evaluation of ct guided neurolysis in the management of pain in infiltrative pelvic m alignancies

Abstract
Aim: To evaluate the efficacy of CT guided neurolytic procedures in the management of pelvic malignancy pain including CT guided chemical lumbar sympathectomy for lower limb pain and CT guided presacral and precoccygeal sympatholysis for pelvic cancer pain. Study design: Prospective study. Materials and Methods: 22 patients in the age group of 30-50 years with infiltrative malignancy were studied Multislice CT guided neurolyticprocedures were done with absolute alcohol through Chiba needle under local anaesthesia .12 patients with lower limb pain underwent Lumbar Sympatholysis.10 patients with pain over sacrococcygeal area and localized perineal pain due to advanced pelvic malignancies,underwent Presacral and Precoccygeal sympatholysis.The intensity of pain was assessed by the visual analogue scale and Graceley’s Verbal descriptor scale before and after the procedure.Patients with pain due to other causes were excluded. Results: 10/ 12 patients of chemical lumbar sympathectomy had greater than 50% of pain relief immediately. At 2 months one patient died, 7/11 patients had more than 50% pain relief.8/10 who underwent presacral and precoccygeal neurolysis had greater than 50% pain relief immediately. At 2 months one patient died, 4/7 patients had more than 50% pain relief. Conclusion: CT guided neurolysis is a highly effective temporary technique to treat pain of infiltrative pelvic malignancies. Side effects were very minimal or rather negligible
