Prevalence of Sub-clinical Hypothyroidismin Type 2 Diabetes Mellitus

Abstract

Background: Diabetes Mellitus and thyroid disorders are two major endocrine diseases that mutually influence each other. The association between thyroid dysfunction and Type 1 DM has long been recognized, although the prevalence of thyroid dysfunction among Type 2 diabetes population is less understood and varies in different studies. This study aims to find the prevalence of thyroid dysfunction, particularly subclinical hypothyroidism in type 2 diabetes mellitus patients. Methods: In this cross-sectional study, 100 consecutive type 2 diabetic patients without any clinical manifestations of thyroid disorders were screened for hypothyroidism,SCH and subclinical hyperthyroidism using serum free T4 and thyroid stimulating hormone (TSH) levels by ECLIA . They were compared with 100 age-and gender-matched healthy control subjects. Results: It was observed that the prevalence of various thyroid disorders was 27% in Type 2DM with a considerably higher proportion of patients with subclinical hypothyroidism (18%).Serum TSH levels were also significantly higher in Type 2 DM( p=0.001) when compared to healthy controls. We also observed a statistically significant positive correlation between the fasting plasma glucose and serum TSH levels (p<0.001). Gender wise distribution showed that women were more prone for thyroid disorders in both the groups with 34.7% as compared to 19.6% in males with sub-clinical hypothyroidism being the commonest . Conclusion: The prevalence of thyroid dysfunction among type 2 DM patients is very high (27 %) with subclinical Hypothyroidism(18%) is being most common. Our observations suggest that patients with Type 2 DM patients be routinely screened for thyroid dysfunctions for better outcomes .
