Evaluation of the comparison of the use of extracorporeal shock wave therapy and ultrasound in patients with a heel spur

Abstract

Background. A calcaneal spur (heel spur) is a degenerative change occurring in the area of the medial process of the calcaneus, which causes the growth of a bony, beak-like structure –an enthes­pohyte. Its occurrence is mainly ascribed to an enthesopathy, or a pathological change of the enthe­ses. The main method of calcaneal spur treatment is physiotherapy, and specifically ultrasound treat­ment. New methods of therapy are presently being investigated and, so far, shock wave therapy has proven most promising. The aim of this thesis was: to evaluate the effectiveness of shockwave therapy in patients suffering from calcaneal spurs in comparison to ultrasound therapy. Material and methods. 40 persons diagnosed with a calcaneal spur took part in the study. The sample was divided into 2 groups of 20 persons each. Group I was the experimental group, treated with shock wave therapy. Group II was the control group, subjected to standard ultrasound therapy. Both groups were studied using VAS and Laitinen scales during the first and the last day of the treatment. Results. On both scales, the patients subjected to shock wave therapy achieved a statistically relevant increase in the effectiveness of treatment in comparison to the control group. A statistically relevant decrease in the intensity and frequency of pain in the experimental group, a significant reduction in medication intake as well as complete relief of symptoms during physical activity after shock wave therapy have been observed. Conclusions. Shock wave therapy is a more effective method of treatment in patients with calcaneal spurs than ultrasound therapy. An increased BMI plays an important role in calcaneal spur formation.<br/><br/>
