Expression of neutrophil elastase in lesional skin is not different in patients with dermatitis herpetiformis and bullous pemphigoid

Abstract
Introduction: Neutrophils most probably degrade dermal-epidermal junction (DEJ) in dermatitis herpetiformis (DH) and bullous pemphigoid (BP) by releasing neutrophil elastase (NE) and MMP-9. Objective: The aim of this study was to compare the intensity of NE deposits assessed using digital microscopic image analysis in lesional skin in patients with DH and BP. Material and methods: NE deposits in 21 DH patients and 24 BP patients were evaluated with immunohistochemistry, intensity of NE deposits was assessed with digital microscopic image analysis and statistically analyzed. Results: Intensities of NE deposits in lesional skin were not significantly different in DH and BP patients. Conclusion: Activation of neutrophils as judged by release of NE is not different in lesional skin in DH and BP, which in turn might suggest that grouping together of DH and BP based just on the criterion of blister localization within DEJ was appropriate.
