Factors Determining the Outcome of Renal Function in Emphysematous Pylonephritis:

Abstract

INTRODUCTION: EPN is a severe necrotizing infection of renal parenchyma and perirenal tissues that is caused by gas producing pathogenic organisms. We conducted a retrospective study at our hospital between aug 2015 and aug 2018 to investigate the factors that determine the outcome of renal function in EPN. METHODS: We retrospectively analyzed the clinical and lab data, imaging findings and outcome of pts with EPN. We studied a total of 28 patients and categorised them as conservative management group (16) and surgical intervention group (12). 8 cases presented with acute pyelonephritis, 9 cases with urosepesis and the remaining 11 patients with multioorgan dysfunction. The overall survival rate was 89 28% RESULTS: Among the risk factors analysed anemia, thrombocytopenia, altered sensorium , higher CT grade, proteinuria more than 3g /L, positive blood culture were significantly associated with mortality CONCLUSION: EPN management requires multidisciplinary approach including hydration and electrolyte management , broad spectrum antibiotics , strict glycaemic control , effective urinary drainage and lastly it may require nephrectomy as a salvage procedure
