Fatal severe pneumonia complicating Chilaiditi syndrome

Abstract
Introduction. Chilaiditi syndrome is the association of radiological (upper-elevation of right diaphragmatic cupola) and clinical manifestations related to hepato-diaphragmatic interposition of the colon or intestine. Most often asymptomatic and discovered during a chest X-ray, it can cover digestive or respiratory complications sometimes fatal. Observation. A 66-year-old woman, diabetic and hypertensive, hospitalized on April 25, 2016 for hypoxemic pneumopathy in an infectious context. The chest X-ray showed ascension of the right diaphragmatic copula associated with right alveolar-interstitial opacities; the chest angio-scan confirmed an association with Chilaiditi syndrome. The evolution was marked by the aggravation of the clinical context despite respiratory infection treatment and symptomatic management of the syndrome then the death occurs on the fifth day of hospitalization. Conclusion. It is important to attract attention to the existence of this syndrome in pneumological practice to facilitate its diagnosis in order to decide an appropriate therapeutic management.
