Haematological profile in neonatal sepsis

Abstract
Background: The clinical features of sepsis are not specific in neonates and a high index of suspicion is required for its identification. The early markers of sepsis like total leukocyte count, absolute neutrophil count, immature cell to total neutrophil ratio, presence of toxic granules in neutrophil, platelet count and C-reactive protein are used in the diagnosis. Aim: To study the haematological profile in neonatal sepsis cases. Material and Methods: The study population consisted of prospectively registered 40 age and sex matched neonates as control and 104 neonates with clinical suspicion of sepsis. These neonates were then subjected to investigations such as, complete blood count, peripheral blood smear stained with eishman stain. Corrected WBC count, absolute neutrophil count, immature to total neutrophil count (I:T) ratio, toxic granules and thrombocytopenia was done.CRP was done by slide latex agglutination qualitative method. Blood culture with antimicrobial sensitivity of the isolates was carried out as per standard protocols. Results: Out of 104 cases, blood culture was positive in 73 cases. Total leukocyte count had sensitivity of 49% and specificity of 65%. Absolute neutrophil count sensitivity of 22% and specificity of 81%. Immature cell to total neutrophil ratio had 67% sensitivity and81% specificity. Toxic granules in neutrophil had 59% sensitivity and 77% specificity. Platelet count had 62% sensitivity and45% specificity, while CRP had 78% sensitivity and 41% specificity. Hemoglobin had 25% sensitivity and 87% specificity, MPV had 87% sensitivity and 27% specificity and PDW had 51% sensitivity and 58% specificity. Conclusion: These sepsis screening tests are simple, cheap, rapid and bedside tests requiring no special laboratory facilities and provide a valuable adjunct in the early detection of neonatal sepsis in small hospitals.
