Hippocampal volumetry and EEG in detection of epileptic focus in temporal lobe epilepsy

Abstract
Focal epileptic seizures of medial temporal manifestation are most common type of human epilepsies. Their morphological base is usually pathological lesion of hippocampal complex. For investigation of their evidence, nature and intensity we have many new and still developed technical methods. One of the most popular is classical EEG test and measurement of hippocampal volume by magnetic resonance modifications. The aim of this study was correlation between localization of pathological electric activity in the EEG examination and intensity of hippocampal volume decrease in T1-weigthed MR volumetry. The study group were 150 adults suffered from drug resistant temporal lobe epilepsy for 16,9 years on average. For the diagnostically important we had define a 5% difference between both hippocamps in one patient. Intensity of pathological electric activity in EEG tests was not dependent of morphological MR volumetric changes. Only the evidence of any anatomical lesion in hippocampal complex in classical MR imaging was positively correlated with degree of hippocampal volume asymmetry. The main conclusion of the study is that it’s no direct correlation between functional activity of temporal lobe and its morphological changes in hippocampal MR volumetry.
