Histiocytic sarcoma with exceptional pulmonary localization

Abstract

Introduction. Histiocytic sarcoma is a very rare, aggressive tumor with a poor prognosis. Resulting from the proliferation of immunoregulatory cells. The use of immunohistochemistry and molecular biology techniques is essential to make the diagnosis and to make the difference with other lymphocyte proliferations, including non-Hodgkin lymphoma. Observation. It is a patient of 71 years, without significant pathological antecedents, who presented for 5 months a dry cough associated with a dyspnea of ​​effort evolving in a context of deterioration of the general state. Cervico-thoracic computed tomography revealed the presence of several cervical and mediastinal lymphadenopathy associated with a focus of condensation in the lower left lobe. Bronchoscopic bronchoscopy showed a diffuse inflammatory appearance with infiltrative stenosis of the left B9 segmental bronchial orifice and whose histopathological study was in favor of histiocytic sarcoma. Conclusion. The low frequency, the mode of presentation and the poor prognosis are all elements that should lead to a better knowledge of histiocytic sarcoma. This will better individualize them in the management compared to non-Hodgkin's lymphoma.
