Histomorphological spectrum of various cardiac changes in sudden death: An autopsy study

Abstract

Background: Ischemic heart disease is the most common leading cause of death worldwide in both men and women. It was reported that concordance between clinical and pathological causes of death are moderate, and the autopsy still provides a very important procedure for evaluating causes of deaths. Over one fifth of clinically unexpected autopsy findings can be correctly diagnosed only by histological examination. Autopsy and particularly autopsy histology are still the most accurate method of determining the cause of death and auditing accuracy of clinical diagnosis, diagnostic tests and death certification. Methods and Material: The present retrospective study was conducted at Department of Pathology during the period of 1st January 2018 to 30th April 2018 in Pt. J. N. M. Medical College Raipur (C.G.), India. 96 specimen of heart were included in the study. Detailed history and post-mortem findings of all cases were collected from the post mortem papers and requisition form. The heart was examined grossly and microscopically to observe various histomorphological changes and findings were correlated, formalin fixed heart were inspected, weight and dimensions of whole heart were recorded. Data was compiled in MS-Excel and it was analyzed using statistical software SPSS v16. Result: Out of the 96 cases studied, majority of the patients were in 41-50 years (23 cases -23.9%) age group, followed by 31-40 years (19 cases -19.8%). There was significant male predominance. In histopathological examination most common finding was atherosclerosis 34 (35.4%) cases followed by myocardial hypertrophy 11 (11.5%) cases, 8 (8.3 %) cases were reported to have myocardial infarction (MI). Among 34 cases of atherosclerotic changes, 16 cases are having calcification and 5cases were present with thrombus in coronaries. Conclusion: Histopathology of the heart is very helpful to determine at a conclusion regarding the cause of death. Such studies give an insight about true prevalence of the diseases as a helpful information in controlling/monitoring disease processes.
