Histomorphological study of nodal lymphoma in a tertiary care hospital

Abstract
Background: Lymphomas are neoplasms of the lymph-reticular system which can arise in the lymph nodes as well as extranodal sites. They can be broadly classified into Hodgkin (HL) and Non-Hodgkin lymphoma (NHL). Nearly two-third of NHL and almost all cases of HL present with non-tender nodal enlargement. Aims: To study the role of Histomorphology and Immunohistochemistry in the diagnosis of Nodal Lymphomas. To study clinicopathological correlation of various lymphomas. Methods and Material: This is a descriptive study done on 60lymph node specimens which were diagnosed as nodal lymphoma at a referral hospital of Mangalore city, between January 2013 and June 2017.The final diagnosis was arrived at after correlating the clinical features, gross and microscopic findings including IHC wherever possible. Results: Out of 60 cases of nodal lymphoma 22(36.6%) were HL and 38(63.4%) were NHL. Mean age was 42.15 years with male preponderance. CHL was diagnosed in 21(35%) cases and NLPHL in 1 case (1.6%). NSHL was the most common subtype (55%) of CHL followed by MCHL (25%). Among 38 cases of NHL, Mature B cell NHL (63.15%) included FL (18.3%) DLBCL (15%), CLL (7.89%), MCL (2.63%) and Mature T cell NHL (15.18%) included ALCL (10.53%), AITCL (5.26%) and Precursor LBL (7.89%). 71% cases of HL showed CD30 positivity in RS cells. All the cases of FL were positive for CD20 and CD10. CD99 was positive in LBL. On follow up, one case of CHL and FL each showed relapse. Three cases of NHL had bone marrow infiltration. Conclusions: Immunophenotypic analysis constitutes an integral part of the work-up of lymphomas as it improves the accuracy of classification and management of the disease. Morphology with collective expression of the immunophenotype, genotype and normal cell counterpart along with clinical details are important for the diagnosis of lymphoma.
