Impact of Bethesda system on reporting thyroid cytopathology

Abstract

The Bethesda system of reporting thyroid cytopathology is a standardised system, improving communication between cytopathologists and clinicians, leading to more consistent management approaches. Fine Needle Cytology (FNC) procedure has emerged as safe, cost effective and reliable technique for diagnosing diseases of the thyroid gland. Methods: We studied all the thyroid cytology cases received between January 2012to December2015 with cyto– histological correlation. Results: Total cases studied were 604. Out of 505(83.6%) cases were benign lesions, 15(2.5%) cases were malignant, 69(11.4%) cases were non- diagnostic/unsatisfactory,11 cases(1.9%) cases were follicular neoplasma/suspicious for follicular neoplasm, 02(0.3%) where suspicious for malignancy and 02(0.3%) were reported as Atypia of undetermined significance. Out of the 80 cases available for histopathological follow-up, cytohistological discrepancies were noted in 5(0.8%) cases. In our study correlation factor was 85% with cytological and histological correlation. Conclusion: TBSRTC is an excellent reporting system for thyroid FNA. The malignancy risk correlates well with previous studies. It provides clear management guidelines to clinicians to go for follow-up FNA or surgery and the extent of surgery.
