Influence of Candida spp. respiratory tract colonization in chronic obstructive pulmonary disease in in-tensive care unit, Colombia, 2010-2015

Abstract

Candida spp. is the most common agent isolated associated to hospital infection of fungal origin in the context of Intensive Care Unit (ICU),Chronic Obstructive Pulmonary disease (COPD), has a particular relevance as a factor asso-ciated for Candida spp infections. the objective of this study was to describe the infl uence of candida spp. respiratory tract colonization in COPD in a Colombian ICU, between 2010-2015. METHODOLOGY: A retrospective case-control study was carried out, including the microbiological records of isola-tes of Candida spp orotracheal samples between 2010 and 2015, the case group consisted of patients with isolation of Candida spp and spirometric diagnosis of COPD, the control group was formed by patients with isolation of Candida spp. We checked the clinical record looking for demographic, clinical, supportive and therapeuthic variables. We des-cribed discrete variables through relative or absolute frequency, while we described continual variables through average and confi dence interval of 95%. RESULTS: we evaluated 116 patients who met the inclusion criteria of the study, according to the studied groups, we obtained 43 patients for cases group and 73 patients in control group. the most common isolated agent was Candida albicans with 76.74% of COPD patients’ isolations and 75.34% of COPD-free patients. we found that COPD patients group was older compared to the COPD-free one, and presented a higher Charlson Index. There was no signifi cant diffe-rence on therapeutic and microbiologic matters. However, there was a higher mortality in COPD patients with 53.49% compared to 20.55% in the other group (p<0.01). CONCLUSION: In our study, patients with COPD accompanied with Candida spp infection presented older age and greater co-morbidities compared to patients with candidiasis but COPD-free. We found that candida colonization cons-titutes an independent risk factor for greater use of corticoids, antibiotics and mortality in the context of COPD patients in ICU
