Abstract
Le but de notre travail est d’analyser les caractéristiques cliniques de la maladie de Behçet dans notre service de médecine interne. C’est une étude monocentrique rétrospective au sein du service de médecine interne à partir des dossiers de patients suivis pour maladie de Behçet. Les critères d’inclusion étaient ceux du Groupe international d’étude sur la MB. Cette série a comporté 343 patients avec 182 hommes et 161 femmes . Le sex-ratio était de 1,13. L’âge moyen de nos malades au moment de la première hospitalisation est de 34.44, avec des extrêmes de 3 ans et de 72 ans.La fréquence des manifestations cliniques était comme suit : aphtose buccale : 100 %, aphtose génitale : 92 %, pseudofolliculite : 85%, atteinte articulaire:65%, atteinte oculaire : 33 %, atteinte vasculaire: 14 %.L’atteinte oculaire, les thromboses veineuses et l’atteinte artérielle étaient significativement plus fréquentes chez les hommes et l’érythème noueux et les manifestations articulaires étaient significativement plus fréquents chez les femmes.Notre série confirme la prédominance masculine de la maladie de Behçet retrouvée dans les autres pays méditerranéens et les pays du Moyen-Orient mais avec une grande présence féminine rejoignant ainsi les séries japonaises et turques. Les fréquences de l’atteinte articulaire et celle oculaire étaient parmi les plus élevées, comme celles des pays méditerranéens. En revanche, l’atteinte vasculaire, digestive, et neurologique était moins fréquente dans notre série.Nous insistons sur le plan thérapeutique sur la grande place de la colchicine à vie et sur le protocole personnel à base d’immunosuppresseurs pour les atteintes oculaires sévères. Summary: The goal of our work is to analyze the clinical characteristics of the disease of Behcet in our service of internal medicine.It is a retrospective monocentric study within service of internal medicine starting from the files of patients followed for disease of Behcet.The criteria of inclusion were those of the international Group of study on the MB.This series comprised 343 patients with 182 men and 161 women only .The sex-ratio was 1,13. The average age of our patients at the time of the first hospitalization is 34.44, with extremes of 3 years and of 72 years.The frequency of the clinical demonstrations was as follows: aphtose oral: 100 %, aphtose genital: 92 %, pseudofolliculite: 85 %, attack articulaire:65%, reached ocular: 33 %, vascular attacks: 14 %.The ocular attack, thromboses venous and the arterial attack were significantly more frequent at the men and the knotty erythema and the articular involvement were significantly more frequent among women. The frequencies of the articular attack and the ocular were among highest, like those of the Mediterranean countries. On the other hand, venous thromboses, digestive, and neurological attack were less frequent in our series.We insist for therapeutic on the great place of colchicines all the life and personal protocol of immunossupressor for sever ocular seatures Mots Cles: -Behçet -uveite -cécité -anevrisme
