Maküla Deliği Nedeniyle Retina Dekolmanı Olan Yüksek Miyop Gözde Maküler Çökertme Cerrahisi

Abstract
Altmış sekiz yaşındaki kadın hasta kliniğimize sağ gözünde bir aydır devam eden merkezi görme alanı kaybı ve görme azalması yakınması ile başvurdu. Düzeltilmiş görme keskinliği (DGK) sağ gözde el hareketleri düzeyinde saptandı. Fundus muayenesinde miyopik korioretinal dejeneratif değişikliklere eşlik eden arka stafilom ve maküler bölgede retinada hafif kabarıklık izlendi. Optik koherens tomografide (OKT) arka kutupta retina dekolmanına eşlik eden maküla deliği ve stafilom izlendi. Olgumuza pars plana vitrektomi, iç limitan membran soyulması, maküler çökertme ve perfluoropropan gaz tamponadı uygulandı. Cerrahi sonrası 3. ayda, olgunun DGK’si 1 metreden parmak sayma düzeyine çıktı ve OKT’de retina yatışıklığıyla birlikte maküla deliğinde kapanma izlendi. Arka stafilomun eşlik ettiği miyopik maküla delikli retina dekolmanlı gözlerin tedavisi oldukça zordur ve tedavisinde çeşitli cerrahi yöntemler tanımlanmıştır. Cerrahiden anatomik başarı sağlansa da fonksiyonel başarı arka kutuptaki korioretinal atrofi nedeniyle beklenenden daha düşük olabilir.
