Mesiodens – classification, diagnosis and indications for treatment Review of a literature

Abstract

Dental abnormalities regarding the number of teeth present as reduced (hipodontia) or increased (hiperodontia) the number of teeth. Additional teeth (eumorphic) have a proper structure (eg. additional premolars). In turn, the supernumerary teeth (dysmorphic) have abnormal anatomy. The most commonly observed supernumerary tooth is mesiodens which incidence in different populations is estimated from 0.05% to 6.4%. Aim. The aim of this paper is to present the classification, diagnosis, indications for surgical and orthodontic treatment and the possible effects of treatment discontinuation. Material and methods. Using PubMed, Science Direct, Polish Medical Bibliography and Wiley browsers papers published until 2016 were selected. The literature search was conducted on the basis of the following keywords: mesiodens, impactions teeth, orthodontic treatment planning. For the analysis, the articles describing issues addressing the aim of work were qualified. Conclusions. Ectopic or delayed eruption of incisors, the presence of pathological diastema wider than 2 mm may suggest the presence of mesiodens. Clinical and radiological examination (photos, dental panoramic X-rays, cone beam computed tomography) allows scheduling the treatment of patients with supernumerary teeth. Treatment of abnormalities associated with the presence of the mesiodens depends on the age, location and morphology of the tooth. Early diagnose and removal of a mesiodens in mixed dentition allows the spontaneous eruption of permanent incisors and often eliminates the need for orthodontic treatment.
