Midgut malrotation with volvulus in an adolescent

Abstract
Midgut malrotation is a congenital anomaly referring to either lack of or incomplete rotation of the intestines around the axis of the superior mesenteric artery during development. It is a very rare presentation in adulthood with incidence of 0.2%-0.5%. Midgut volvulus is a complication of malrotation in which clockwise twisting of the bowel around the Superior Mesenteric Artery axis occurs because of the narrowed mesenteric attachment. The clinical diagnosis of midgut volvulus in adults is difficult because the presentation is usually nonspecific and malrotation is rarely considered. We present a case of malrotation of midgut with partial volvulus in an adolescent female patient who presented with intermittent abdominal pain, nausea and vomiting since six months. The patient underwent abdominal ultrasound, Upper gastrointestinal contrast studies and contrast enhanced computed tomography. Although small bowel follow-through is often enough to recognize the type of malrotation, using multimodality imaging may offer a better definition of this abnormality with a better definition of the kind of malrotation, by adding additional anatomical information
