Modern concepts on the etiology of the „idiopathic” ventricular arrhythmias in children

Abstract

Ventricular arrhythmias occur before and after surgery for congenital heart defects, in children with cardiac tumors, hypertrophic or dilated cardiomyopathy, arrhythmogenic right ventricular cardiomyopathy, myocarditis, genetic channelopathies syndromes, electrolyte or metabolic imbalance. “Idiopathic” ventricular arrhythmias which occurs in the absence of identifiable functional or structural heart disease are the biggest and very controversial group. Until now the diagnosis of “idiopathic” character of arrhythmia was based on the absence of the demonstrable heart disease in x-ray and echocardiographic examinations, absence of electrocardiographic markers of channelopathy and no signs suggesting inflamatory process in the myocardium. Recently with the introduction of other noninvasive (myocardial scintigraphy using technetium labeled antigranulocytes antibodies, cardiovascular magnetic resonance imaging) or invasive examinations (endomyocardial biopsy) in part of cases of “idiopathic” ventricular arrhythmias it was possible to demonstrate “silent” form of myocarditis, early stage of dilated cardiomyopathy or arrhythmogenic right ventricular cardiomyopathy. It changes the methods of treatment of the patients and may worse the prognosis.
