ORO-ANTRAL COMMUNICATION (theoretical course)

Abstract

 Anatomical particularities of maxillary sinus, teeth and correlation between this two components represents one of the main causes of oro-antral communication occurrence (appearance) after surgery at the superior alveolar process. The oro-antral communication it the accidental opening of the maxillary sinus during surgery, without signs of inflammation. The oro-antral communication can occur only in the extraction of uninfected tooth. The establishment of oro-antral communication in tooth extraction must by treated as a perforative sinusitis. The oro-antral communication symptoms depend on the elapsed time after the appearence of the communication, the presence or the absence of foreign elements. Mandatory clinical signs are the passage of air flow the maxillary sinus cavity in the oral one and vice versa, the flow of fluids trough nasal fossa from oral cavity, the removing of blood trough nasal fossa after the tooth extraction (one the one side). Para clinical investigations will depend on the specific clinical case and wil very from case to case. Necessarily will begin with radiologic examination. It can be realized (made) using x-ray, retro alveolar x-ray is required and axial incidence x-ray, orthopantomogram, CT. The most frequently used method in maxillo-facial surgery clinics is the oro-antral communication plasty with buccal flap and palatal flap. It is obligatory to inform the patients about oro-antral communication. The method of oro-antral сommunication treatment muast be adjusted to every a particular clinical case.
