RATIONAL CHOICE OF ANTIBIOTICS FOR WOUND INFECTION: A GUIDE FOR THE CLINICIANS

Abstract

 BACKGROUND Wound infections are one of the most common Health Care Associated Infections (HAIs) and are an important cause of morbidity and mortality that leads to prolonged hospital stay and increased cost of hospitalisation. Surveillance of wound infections with feedback of appropriate data to the physicians is desirable to start early empirical therapy and to reduce the rate of infection. OBJECTIVES Determination of the commonly encountered pathogens in pus samples along with their antibiotic susceptibility pattern. MATERIALS AND METHODS This study was conducted from January to December 2014 in a tertiary care hospital in Kolkata. Pus samples received in the Department of Microbiology were processed and identified according to standard protocols. Antibiotic susceptibility test was done by Kirby Bauer disc diffusion method. RESULT Out of 2179 pus samples, 1679 (77.05%) cases yielded positive culture results, while 500 (22.94%) samples produced no growth. Among the 1679 culture positive pus samples, 993 (45.57%) were Gram negative bacilli and 686 (31.48%) were Gram positive cocci. Klebsiella pneumoniae was the commonest isolate followed by Staphylococcus aureus. Other important bacteria isolated were E. coli, Pseudomonas aeruginosa, S. epidermidis, Acinetobacter spp., Citrobacter spp., S. pyogenes and Proteus spp. The Gram positive isolates were mostly susceptible to Vancomycin and Linezolid, whereas amongst the Gram negative isolates, the most effective drugs were Imipenem and Piperacillin-Tazobactam. CONCLUSION Changing antimicrobial resistance pose a challenge in treating pyogenic infections. Recognition of the resistance pattern of the potential pathogens is essential in formulation of an effective empirical antimicrobial regime.
