New research trends on prevention of embolic complications in patients with atrial fibrillation. Participation of the Department of Cardiology Postgraduate Medical School in multicentre clinical trials

Abstract

Atrial fibrillation (AF) is the most common supraventricular cardiac arrhythmia. AF is also associated with increased cardiovascular mortality and morbidity. Incidence of both AF and stroke increases with age.Stroke in patients with AF is more severe and more likely to be fatal. Prevention of thromboembolism with oral anticoagulants is the main therapeutic strategy for patients with AF. Vitamin K antagonists (VKA), which reduce the risk of stroke in patients with AF, are underutilized. VKA despite their high effectiveness in the prevention of embolic complications significantly increase the risk of bleeding. There is a need for new, effective, safe and easy to use anticoagulants. Recently published studies provide, very encouraging results. The novel direct thrombin inhibitor dabigatran showed stroke reduction and improved safety compared with VKA agents. Other ongoing studies with anticoagulants will assess the effectiveness of new agents and help to identify those patients with AF who are likely to benefit from novel therapy. Our department participated in few of these studies.
