Abstract
Pulmonary tuberculosis is a major public health problem. In its common form, diagnosis is usually easy, but it can be presented in unusual form and causes diagnostic and therapeutic delay. We report a case of pseudo-tumoral pulmonary tuberculosis. This is a 57-year-old patient, smoker, complaining of left chest pain with decreased overall condition. The chest x-ray showed a heterogeneous oval left apical opacity with irregular contours. The biological evaluation was without anomalies. BK researches in sputum were negative on direct examination. Bronchial fibroscopy was normal. The thoracic CT scan concluded to a pulmonary nodule with a diameter of 2 cm and spiculous contours in the seat of culmina. The pulmonary nodule had a homogeneous density, enhanced after injection of contrast agent and it is associated with signs of lung fissure attraction. Multiple bilateral mediastinal lymph nodes were noted. Given to the age, smoking exposure and radiological features of the nodule, malignant origin was suspected. A surgical biopsy for diagnostic and therapeutic purposes has been proposed. The peroperative exploration has found a nodule of 3cm at the level of culmen. A biopsy wedge was performed. The extemporaneous examination and the final pathological examination concluded to a tuberculoma. Through this observation, we emphasize that pseudo-tumoral pulmonary tuberculosis is a particular form of tuberculosis by its radio-clinical presentation, which can mimic bronchopulmonary cancer.
