Odontoma in a mother and son

Abstract

Odontoma is a developmental defect of dental hard tissue. It forms as a result of a pathological division of dental lamina. It is usually comprised of an enosseous cluster of small tooth-like structures. The presence of odontoma may lead to disturbances in the tooth eruption process, incorrect tooth positioning or the retention of teeth in the bone. This paper describes two cases of odontoma located in exactly the same position in a mother and son. The documentation of two related patients (mother and son) included diagnostic casts and X-rays of the patients. In both cases the odontoma was located in the area of the right upper central incisor. In son a persistent deciduous incisor, an impacted permanent incisor and co-existing Class II div. 2 malocclusion was found. The treatment consisted in the extraction of persistent deciduous tooth 61 and removal the odontoma, followed by orthodontic movement of permanent incisor 21 into the correct position in the dental arch. In the case of the mother an impacted permanent tooth (21) and lost teeth in the posterior segments. The treatment in her case consisted in the removal of the odontoma and the impacted permanent tooth, and in the prosthetic restoration of missing teeth. A clinically detected missing permanent tooth, a retained deciduous tooth, and a distended alveolar ridge should all be regarded as indicators of possible pathology and therefore need to look for the origin of the problem. The occurrence of dental anomalies located in identical positions in the related patients, in this case in a mother and son, suggest that the origin of this problem was hereditary. Radiographic examination plays an important role in the detection of odontoma, and it should become a routine procedure in every patient attending a dental surgery. (Orthod. Forum 2011; 7: 192 - 7).
