Open reduction and internal fixation for displaced diaphyseal fractures of the clavicle with anatomical Locking Compression Plate- A prospective study

Abstract

Background: Mid-shaft clavicle fractures (MCFs) account for approximately 80% of all clavicle fractures and 30% of these fractures are sports-related injuries in young males. Plate fixation can ensure firm anatomical reduction in severe displaced or comminuted fracture of the clavicle compared to conservative management. Objective: To assess the functional outcomes, the complications associated and the duration of union while treating displaced mid-shaft clavicular fractures by open reduction and internal fixation using locking compression plates. Materials and Methods: The prospective observational study was conducted at the Department of Orthopaedics, Sri Venkateshwaraa Medical College Hospital and Research Centre, Ariyur, Puducherry from April 2011 to May 2016 by following up 200 individuals with displaced mid shaft clavicular fractures who underwent surgery for anatomical reduction of fracture using locking compression plates (LCP) from the time of admission till there was documented radiological evidence of bone union.. Results: Majority of the cases (82%) united by the end of 10 weeks and between 10 – 12 weeks (16%). There was one case which went for non-union. Average union time was 9.2 weeks. Functional outcome as assessed by Constant and Murley score was excellent for 128 (64%) and good for 72 (36%) patients. Conclusion: Management of displaced mid-shaft clavicular fractures with pre contoured locking compression plate showed good clinical outcomes in terms of bone union and functional rehabilitation.
