Medial Opening Wedge High Tibial Osteotomy By Hemicallotasis Using Self Aligning Dynamic Fixator

Abstract

Back ground: High Tibial Osteotomy (HTO) is a widely performed procedure for Osteoarthritis of the knee joints. It is the choice of treatment in medial compartment of osteoarthritis of knee joint in young and physiologically active patients. The advanced surgical techniques and improved instrumentations has given promising results with least complications. Aim: To assess the outcome of medial opening wedge high tibial osteotomy by hemicallotasis using self aligning dynamic fixator. Methods: This is a prospectively collected data of patients with medial compartment osteoarthritis of knee joints treated with medial opening wedge HTO by hemicallotasis using self aligning dynamic fixator. The patients were evaluated with pre-operative full length radiographs. All patients were evaluated with arthroscopy before HTO. The patients were followed up at regular intervals; radiographs were assessed for callus formation and union. The results were assessed with visual analogue scale (VAS) and Knee Society Score . Statistical Analysis: Paired T test, Wilcoxon Signed Ranks Test. Results: A total of 30 knees in 25 patients with medial compartment osteoarthritis with varus were operated for HTO using dynamic axial fixator between 2010 and 2015 at our hospital. There were 15 male and 15 female knees. Mean age of patients was 52.3 years. mean preoperative hip-knee-ankle (HKA) was 170.370 and it improved to 180.4 0 post operatively at mean follow up period of 18 months. Mean pre operative pain score was 7.47 at it improved to 1.70.Mean preoperative knee score 36.23 and functional score was 41.6 which improved at final follow up to 88.87 and 81.3 respectively. All these results found to be statistically significant. Conclusion:HTO with dynamic external fixator avoids many complications associated with other methods and gives comparable results at a much lower morbidity and cost. It gives accurate correction of the deformity as correction can be controlled even during post operative period. The stability of fixation allows early mobilization and weight bearing.
