Operational capabilities of the COVID-19 case definitions to detect patients diagnosed by RT-PCR

Abstract
Objetive. The operational case definitions (OCD) for the new coronavirus (COVID-19) were constructed to identify potentially infected persons (PIP), in this investigation we measured the operational capabilities of its components to discriminate COVID-19 persons infected. Methodology. A cross-sectional design was performed using data contained in the mandatory notification records and PPI medical records of COVID-19 between March-May / 2020, attended by the IPS Red Cross in Bogota, the sociodemographic and clinical characteristics were described, and determined the operational capabilities of OCDs and its components to discriminate patients diagnosed with COVID-19 by RT-PCR. Results. 2174 PIPs of COVID-19 were identified, the prevalence of those infected was 14.7%, median age 36 years, 57.7% women, the majority presented close contact, between 12.7-21.1% of those infected were health workers and 35.2% were asymptomatic carriers. Those infected more frequently presented fever (32.8%; p: <0.001), and in general, 50-60% presented respiratory symptoms. The sensitivity of the clinical manifestations was less than 50%. The components of the OCD most sensitive and specific to discriminate infected with COVID-19, respectively, were the history of close contact and trips to areas of local community transmission in the last 14 days. Discussion. The low sensitivity of clinical manifestations to discriminate patients infected with COVID-19, leads to update the OCD to improve PIP identification. Additionally, measures to prevent the spread in emergency care settings should be optimized and include a routine screening in Non-COVID rooms.
