Orthodontic extrusion as a method of choice in cases of subgingival root fracture – long-term observation

Abstract
Introduction: Traumatic dental injuries to the anterior region create a therapeutic problem particularly when subgingival root fractures are concurrent with coronal fractures. Aim of the study: To describe multidisciplinary management and long-term evaluation of cases of subgingival root fracture of maxillary incisors. Materials and methods: Two patients who presented with subgingival root fractures were treated with fixed orthodontic appliances to exert forced radicular eruption. This process lasted up to six months. Following stabilization gingivoplasty was performed around the extruded root. Post insertion and temporary crown concluded treatment. When skeletal development was complete, a porcelain-fused-to-metal crown was prepared. Results: Orthodontic treatment allowed to retain the natural root system of subgingivally fractured teeth and to make a suitable prosthesis. Conclusions: Weak orthodontic forces similarly to physiological eruptive forces cause tooth and bone migration. Forced orthodontic eruption of subgingivally fractured roots of anterior teeth creates favourable conditions for prosthetic restorations.
