Osteopenia related sacral insufficiency fracture – diagnose and clinical course

Abstract

An insufficiency fracture of the sacrum in a state of disturbed bone mineralisation (osteopenia, osteoporosis) is a rare injury of the axial part of the motor system. It involves chronic pain in the lower back, the hip joint and the buttocks. These symptoms can lead to an incorrect diagnosis of spine and hip joint degeneracy, masking the real cause of the disorder. A general radiological examination of the pelvis usually does not reveal damage to the sacrum. The aim of this study is to present the diagnosis and treatment in a patient aged 77 years with an insufficiency fracture of the sacrum, who had chronic pain in the lumbar sacral spine, groin and the right buttock, without trauma in anamnesis and who underwent a cementless total replacement of the right hip joint using a Corail implant a long time before the study. The aim of the study was achieved based on an analysis of the patient’s clinical state, diagnostic imaging and and case history.
