Abstract

Ovarian reserve determines the functional potential of ovary and reflects the quantity and quality of oocytes within it. As the number of in vitro fertilization (IVF) pregnancies is on the rise, it has become imperative for gynaecologists to understand the concept of ovarian reserve. Association between the age of woman and reduced fertility is well documented. Apart from chronological age, ovarian reserve depends upon various other factors. Ovarian reserve tests can be classified as static and dynamic tests. Commonly used static tests are measurement of basal serum follicle stimulating hormone (FSH), serum anti- Mullerian hormone (AMH) levels and day 2-5 antral follicle count (AFC) on ultrasound. AMH has now emerged as a novel marker of ovarian reserve. Ovarian reserve tests (ORTs) have modest predictive properties and should only be used for screening/counseling purposes. Main purpose of ORTs is to identify women with diminished ovarian reserve and accordingly decide the appropriate stimulation protocols. This review article will highlight the concept of ovarian reserve, commonly used ovarian reserve tests and management of women with diminished ovarian reserve.
