Abstract

Acute lymphoblastic leukemia (ALL) is the most common hematological malignancy in children. L3ALL or Burkitt’sleukemia is less common; its clinical manifestations that lead to the diagnosis usually reflect the medullary and extra-medullary infiltration by the disease.Burkitt's lymphomas and leukemias were considered for a long time to be a very poor prognosis because of a very high incidence of early relapses.The medullary attacks are exceptional. We report a rare case of Burkitt’sleukemia revealed by pancytopenia in a young 15 years child. A 15-year-old child, with no notable pathological history, who for 4 months has had incoercible postprandial vomiting associated to asthenia.An OGD was performed in favor of HP gastritis treated without clinical improvement, with polyarthralgia of the large joints, biologically the hemogram had shown pancytopenia.Amyelogram had found a 42% infiltration of undifferentiated blasts MPO-negative and immunophenotyping in favor of mature Burkitt-type acute lymphocytic leukemia, Cytogenetics had founded a clonal chromosomal abnormality involving the long arm of a chromosome 14 14q32 with FISH: Presence of a rearrangement of the Ig H locus in 14q32. The patient was put on protocol Burkitt lymphoma group II. Despite their exceptional characteristics, leukemia in Burkitt must be recognized among the differential diagnoses of ALL. The obvious hematological context must evoke the diagnosis and a myelogram + immunophenotyping and karyotype must be carried out without delay because the spontaneous evolution is quickly fatal.
