Parotid gland tuberculosis after chemoprophylaxis

Abstract
Introduction. Parotid gland tuberculosis is uncommon and with no specific clinical presentation. We present a case of parotid gland tuberculosis that occurred after antituberculosis chemoprophylaxis in patient treated with anti TNF-alpha for a crohn’s disease and we discuss this pathology’s elements of diagnosis and the tuberculosis reactivation after chemoprophylaxis. Case report. We report a 58-year-old patient with a history of crohn’s disease that received a chemoprophylaxis based on isoniazid for nine months before starting anti TNF-alpha treatment. The influximab treatment remained unsuccessful, and then the patient was placed under a combination of azathioprime and adalumimab. Two years later, he presented with a parotid glands and cutaneous tuberculosis. Conclusion. This case shows that tuberculosis can manifest after chemoprophylaxis in patients treated with anti TNF-alpha. Therefore, we should be aware during and after immunosuppressive therapy.
