Percutaneous treatment of iatrogenic femoral artery pseudoaneurysms

Abstract
Introduction. Percutaneous examinations and procedures are widely used in cardiology and interventional radiology. They enable accurate diagnosis and intravascular treatment of vascular diseases.Percutaneous procedures are low invasive and carry out lower risk of complications, as compared to cardiac or vascular surgery procedures; therefore, their number is constantly increasing. One of the most common complication of those procedures is pseudoaneurysm of femoral artery located in the region of puncture. The occurrence of this complication is 1 to 8%. Pseudoaneurysms can be treated surgically or by percutaneus thrombin injection to their sacs.Aim. The aim of the present study was to evaluate the efficacy of treatment of pseudoaneurysms, complicating intravascular procedures from the femoral artery access, using percutaneous thrombin injection.Material and methods. Iatrogenic pseudoaneurysms were treated with thrombin injection procedure in 36 patients. Each patient received 100-800 m.u. of thrombin guided by ultrasound in Duplex Doppler option.Results. Embolization of pseudoaneurysm was obtained in 100% of cases, in 97% there was an immediate success, in one case additional compression by an ultrasound probe for a few minutes was needed. We avoided thrombotic and allergic complications due to correct qualification to this treatment.Conclusions. Percutaneus administration of thrombin is safe and efficient method of treatment of iatrogenic pseudoaneurysms. Qualification to administration of thrombin should be based on morphology and the blood flow in the sack of pseudoaneurysm.
