The retrospective evaluation of uterine fibroids removal through laparoscopy or conventional laparotomy

Abstract
Objective. The retrospective study comparing the efficacy and complications comparing myomectomy by laparoscopy and laparotomy. Materials and Methods. From March 2009 to April 2010 in I Department of Gynaecology and Obstetrics, The Medical Center of Postgraduate Education in Warsaw, 22 patients were admitted because of fibroids. I group was referred to laparoscopic myomectomy (17 patients – 77%) and II group was referred to myomectomy by laparotomy (5 patients – 23%). 3 laparoscopies were converted to open procedure due to difficulties of hemostasis. Results. I group: the mean diameter of myomas 72.8 mm (min. 30 mm, max. 120 mm), II group: the mean diameter of myomas 55 mm (min. 45 mm, max. 80 mm). The number of fibroids in both groups (2, max 6 vs 5.8, max. 14). The blood loss was compared in both groups: anaemia was reported in 21.4% in I group vs 50% in II group. Surgical time in laparoscopic myomectomy was compared to open myomectomy (108 vs 85 minutes). Shorter hospitalization was in I group following laparoscopic myomectomy (5.4 days vs 6.4 days). Conclusions. Laparoscopic myomectomy is a safe procedure in the hands of an experienced advanced laparoscopic gynaecologist. The localization and the histology of fibroids do not influence to the method of treatment. The multiplicity of changes may impact to the choice of technique. The hospital stay is shorter and the rate of complications is lower after laparoscopic myomectomy. The surgical time is slightly lower with abdominal myomectomy, but the difference is not significant.
