Precontoured Plate Fixation Restores Early Shoulder Function With Minimal Complications In Displaced Midshaft Clavicular Fractures

Abstract
Background: Plate fixation of displaced midshaft clavicular fractures has been shown to decrease nonunion, symptomatic malunion, and residual shoulder disability compared with nonoperative treatment. To this purpose we evaluated functional, clinical and radiological outcome of patients undergoing operative treatment by locking plate for midshaft fractures of clavicle. Methods: Twenty patients with a mean age 32±6 years having fresh fracture of the clavicle participated in our study .All patients were operated with locking plate & screws for middle third clavicle fracture under general anesthesia. Radiographs were reviewed to identify radiographic union time, malunion, and nonunion. Pain was subjectively assessed with a visual analog scale. Complications were recorded. Shoulder function was assessed using the Constant Score System (CSS). Results: We report that 95% of patients had improves shoulder function after precontoured plate fixation in displaced midshaft clavicular fractures. 82% (14/17) of patients with Robinson type 2B1 fractures had excellent functional scale where as 33.3% (1/3) with type 2B1 fractures had such an outcome. Type-2B2 fractures required significantly more union time when compared to type 2B1 fractures. Three patients had hypertrophic skin scar and in two had plate prominence. One delayed union and one hardware loosening resulting in nonunion. No patient has turned up for implant removal at the end of one year. Conclusions: Precontoured plate fixation improves shoulder function in 95% of patients in displaced midshaft clavicular fractures with minimal complications.
