Preoperative predictors of mortality in adult patients with perforation peritonitis: A review

Abstract
Despite advances in surgical techniques, and intensive care support, management of peritonitis continues to be highly demanding, difficult and complex. Age, sex, site of perforation, hypoglycemia, renal dysfunction, the duration of symptoms and delay in the surgical treatment have been reported as the determinants of mortality in patients with perforation peritonitis and sepsis. Various scoring indices such as the APACHE score, SAPS, Boey Score, Multi Organ Failure (MOF) Score, Mannheim Peritonitis Index (MPI) and Jabalpur Index have been introduced to stratify the risk in such patients. This article reviews the various risk indices specific for perforation of the proximal gastrointestinal tract. Apart from MOF Score, the MPI and the Jabalpur Index, none is specific for perforation peritonitis alone. Moreover, while the indices used in intensive care and sepsis are tedious and difficult to implement, the others mentioned above incorporate the intraoperative and postoperative status as well.
