Abstract
Neck lymph node metastases of squamous cell carcinoma of the upper aerodigestive tract are the most important prognostic factor in head and neck cancers. They worsen by 50% the 5-years results of treatment. The frequency of lymph node metastases depends on location of the primary tumor and various clinical and pathological factors. The diagnosis of lymph node metastases is made by clinical assessment of the neck (palpation) as well as by ultrasonography (USG) and computed tomography (CT scan). The most precise information on metastasis is achieved through serial histological examination of the lymph nodes excised during the neck dissection: the diagnosis of micro-metastases is possible. A very important and difficult problem is caused by metastases in cNO nodes. There are various opinions on treatment modalities in these cases – elective neck dissection seems to be the most effective. In cases classified as cN+, a therapeutic neck dissection is performed: radical, selective or radical modified.
