Abstract
The alimentary tract of a healthy human being is a special ecosystem and stomach with its acid environment occupies a unique place in this system. According to the statistical data of last ten years, about 5-10 % of population in developed world is suffering from Chronic Gastritis and Peptic Ulcer. These diseases are among main diseases also in Georgia, especially in Samegrelo Region. They affect both men and women almost with the same frequency. According to the research of both foreign and our scientists of last ten years, Helicobacter pylori plays crucial role in the etiology and pathogenesis of these diseases. There is a link in 85-90 % of cases. Due to action of H. pylori defense mechanisms of mucous membranes deteriorate and aggressive factors increase. At the International Congress of Gastroenterologists (Australia, 1990) so called “Sydney Classification” was adopted for different types of Gastritis. This classification names a special form of Gastritis – H. pylori-associated Chronic Gastritis or “B Gastritis”. European Research Group for H. pylori and distinguished European gastroenterologists adopted so called “Maastricht Agreement” in 1996 (Maastricht, Netherlands). This agreement was principally new and it covers essential aspects of H. pylori eradication. The etiology of Chronic Piloro-Antral Gastritis is completely It is true that the role of infection in etiology of Peptic Ulcer is doubtless, but up to now the experimental model of Peptic Ulcer does not exist; development of ulcer does need presence of other co-factors; H. pylori is a semi-pathogenic microbe which is often accustomed to life in acid gastric environment. During evolution long time ago a human being and H. pylori met each other and in contrast to other microbes it managed to adapt to the living conditions in acid gastric environment. It colonized gastric epithelium and built a symbiotic relationship with the human being; Peptic Ulcer should be regarded as a gastroenterological and not an infectious disease and H. pylori should be considered as an important factor, especially as a local important factor; H. pylori or any other infectious agent cannot be a sole cause of Peptic Ulcer. This microbe can reveal its pathologic nature only if gastric mucosa is damaged by other factors; Epidemiological as well as experimental data show that one of the causes of Chronic Gastritis and Peptic Ulcer among others is alimentary factor.
