Reliability of the results of drug susceptibility testing Mycobacterium tuberculosis as an important factor of supervised treatment of tuberculosis

Abstract

Introduction. The discovery and introduction antituberculous drugs to the therapy in the second half of XX century, became a turning point in the history of tuberculosis. Unfortunately, this has resulted in closely associated with the use of drugs, the phenomenon of rising drug resistance among M. tuberculosis strains. Lack of standardized methods for susceptibility testing causes difficulties in correctly identifying resistance phenotype. These tests are performed without laboratory quality control may show erroneous results. The phenomenon of drug-resistant TB strains has necessitated the introduction and development of rapid diagnostic methods.The aim of this study was to evaluate the usefulness of automated Bactec MGIT 960 system for determining the phenotype of resistance to the four basic drugs (RMP, INH, SM, EMB) among Mycobacterium tuberculosis strains.Material and methods. The present analysis is based on the results of susceptibility testing of 63 M. tuberculosis strains isolated from patients at IGiChP or other polish centers where tuberculosis is diagnosed. Drug susceptibility testing was performed on solid medium (L-J) by proportion method and liquid medium using the Bactec MGIT 960 system.Results. Compatibility of drug resistance results for all drugs in both methods was 94.8%. The highest compatibility of the results on solid medium (L-J) and Bactec MGIT 960 system obtained for isoniazid and rifampicin – 62/63 marks (98.4%).The time waiting for the results of susceptibility testing of liquid medium ranged from 4.6 to 12.8 days (average 8.95 days), and of solid medium from 28-42 days (average 36.67 days) (p < 0.001).Conclusions. The Bactec MGIT 960 system allows to get rapid and reliable results of drug susceptibility testing of M. tuberculosis strains. This system reduces the waiting time for results, and allows faster including the correct treatment of the patient.
