Role of serum amylase, serum lipase and MCTSI in the diagnosis of acute pancreatitis

Abstract
Aim: To find out the Incidence of pancreatitis presenting in our set up and accurate diagnostic modality for the diagnosis To compare various prognostic indicators in the Management of Acute Pancreatitis, Method: Total 50 patients cases of acute pancreatitis were studied prospectively. Each one of the patients from Yenepoya Medical College and hospital from the year 2015 to 2017 were assessed clinically, biochemically and radiologically and the outcome was tabulated. Results: Numbers of cases of acute pancreatitis under the MCTSI score of 0-2 were 31, out of which the mean value of Serum amylase is 423 and Serum lipase is 481. MCTSI score range of 3-5 the value of serum amylase is 827 and lipase is 1135. MCTSI score of 6-7 the serum amylase value is 2515 and lipase is 3608, MCTSI score of 8-10 the mean value of serum amylase is 1298 and lipase of 1586.The mean value of serum amylase for 50 patients suffering from acute pancreatitis was 768.34. And the mean value of serum lipase was 982.1.Out of 50 patients suffering from acute pancreatitis, 12 patients (24%) had a MCTSI Score of 1 and 2. 7 patients (14%) had a MCTSI score of 1. 5 patients (10%) had a MCTSI of 3 and 8 respectively. 4 patients (8%) had a MCTSI score of 4. 2 patients (4%) had a MCTSI score of 6 and 7 respectively. Out of 50 male patients 100% of them were alcoholic, 7 (14%) were with gall stones and 43(86%) were without. Out of 26 female patients, 25(98%) of them were non alcoholic and 1(2%) was an alcoholic, and 23 (88%) of them gall stones and 3 (12%) did not. Conclusion: MCTSI showed that most common score is 1 and 2 in our setup and serum amylase, serum lipase values were not specific as it was moderately increased. The correlation in severe cases with the MCTSI of 8 to 10 the amylase and lipase value ranged from 30 to 4000s with a mean value of 1298 and 1586 respectively. The common factor in males in our setup was alcohol whereas in females it was gall stones.
