Role of Ultrasound in the Management of Developmental Dysplasia of Hip with Pavlik Harness

Abstract

Background: Ultrasonography has been accepted nowadays as a useful imaging modality in the early detection and monitoring treatment of developmental dysplasia of the hip with the Pavlik harness. Graf’s method of angle calculation suggests severity of dysplasia present in the affected hip. e purpose of this study was to evaluate the results of Pavlik harness treatment in correlation to Graf’s criteria of the ultrasound examination. Materials and Methods: The study was carried out at our pediatric orthopedic unit of a tertiary care hospital from August 2009 to December 2013. e alpha angles and beta angles of 27 hips in 25 patients were measured through ultrasound and classied according to the Graf‘s classication and treated with Pavlik harness. Results: Out of 25 patients, 20 (80%) were successfully reduced aer the use of the Pavlik harness. e average duration of treatment was 15 weeks. e average age at the start of treatment was 3 months. ere were 11 patients in IIb, 09 in IIc, 03 in IId, and 02 in III. When we compared different Graf types regarding outcome signicant difference was (P = 0.002) found. Conclusions: Graf’s classication in different categories is a valuable marker of hip dysplasia. Prognosis and treatment outcome with the Pavlik harness can be easily predicted based on Graf’s criteria.
