SERUM LIPID CONCENTRATIONS IN PRE-ECLAMPSIA AND NORMAL PREGNANCY - A COMPARATIVE STUDY

Abstract

The association between pre-eclampsia and atherosclerosis is a much discussed topic. The changes in lipid profile observed during pregnancy have significant effects on woman’s long term health, because elevated serum triglycerides is an independent risk factor for coronary artery disease. The aim of this study is to assess the lipid profile in pregnancy with pre-eclampsia and to compare that with normal pregnancy. METHOD: The study group consisted of 150 subjects in three groups of 50 each, selected according to the inclusion criteria. The groups were normotensive pregnant women, those with mild pre-eclampsia and severe pre-eclampsia. Blood samples were collected from all the subjects after 12hours of fasting, which was centrifuged at 3000rpm for 10 minutes and analyzed for serum cholesterol, serum triglycerides, serum HDL, LDL and VLDL using an automatic analyzer. Data was analyzed using Chi square test and ANOVA using SPSS software version 16.0. A p value of <0.05 was considered significant. STUDY DESIGN: Cross sectional study. STUDY SETTING: Department of Obstetrics and Gynaecology, Govt. Medical College, Kozhikode, Kerala, India. RESULTS: In this study, 48.7% of the subjects were in the age group of 18-25 years and 17.3%, above 30 years. The total cholesterol level was high in 54.7% of the entire subjects. The highest distribution were in the severe pre-eclampsia group with a significant p value of <0.001. The triglyceride values were high in 18% of normotensive subjects, while it was 46% and 80% in the mild and severe pre-eclampsia groups respectively. HDL values were normal in the entire normotensive group. In the mild and severe pre-eclampsia groups, 96% and 86% had normal HDL values. High LDL values were seen in 8% of normotensive group, but it was 32% and 28% in the mild and severe pre-eclampsia group respectively, with a significant p value of <0.001. VLDL level was high in 18% of the normotensive group, while it was 46% and 80% in mild and severe pre-eclampsia groups. CONCLUSION: This study shows that women with pre-eclampsia have abnormal lipid profile, which is a significant risk factor for coronary artery disease. Those with abnormal lipid profile should receive adequate counseling to adopt healthy habits and life styles and to seek periodic medical checkups for early detection of coronary artery disease.
