Severe allergic reaction during orthodontic treatment - a case report

Abstract
Orthodontic treatment of patients with medical history of atopic dermatitis and/or allergic reaction to orthodontic appliance components puts both clinician and patient at risk of allergic manifestation occurence during different stages of treatment. Application of hypoallergic materials or the ones that contain no allergen at all, lowers this risk but never completely rules it out. Allergic manifestation in orthodontic patients is often an effect of synergic reaction of more than one allergen. Allergy to nickel is ten times more common in female than in male patients; this fenomenon being directly connected with frequent use of artificial jewellery, chemicals in the household and cosmetics by women. The aim of this paper is to present a case of 39-year old patient, where severe allergic reaction with the signs of anaphylactic shock led to premature removal of orthodontic appliance. Diagnostic examination carried out by means of patch test showed hypersensivity to both metal and ceramic elements of appliance. It is clearly stated in the literature review, that patients, with medical history of allergic reactions to substances contained in elements of fixed appliance, are at higher risk of complications during orthodontic therapy. Age of patients or incidence of allergy in family history have no impact on the frequency of allergic manifestation during treatment. (Orthod. Forum 2012; 8: 143-149).
