Spontaneous transvaginal evisceration of small bowel

Abstract
Evisceration of small bowel through vagina is a rare complication that can occur in posthysterectomy patients or sexual assault/trauma patients. We present a case of spontaneous intestinal prolapse through vagina in a 86-year old female. Six years prior to the prolapse, the patient used vaginal pessary as a nonsurgical method of treatment of pelvic organ prolapse, but one year later, the pessary was removed during one of gynaecological follow-ups. On admission to hospital eviscerated small bowel loops were irreducible, thus the patient was directed to the general surgery ward and qualified for emergency surgery during which bowel loops were reduced to the peritoneal cavity. The defect in posterior vaginal vault was closed by two layers of running sutures. Additionally, the vagina was fixated to the uterus. Bowel loops were viable, with no signs of necrosis of small bowel thus no resection was needed.
