State of the hepatobiliary system in boys with hypoandrogenism

Abstract
Objective — to study the state of hepatobiliary system in boys/youths with delayed puberty. Materials and methods. The study involved 37 boys and youths with hypoandrogenism, aged 13 — 18 years, who were examined in the endocrinology department of the SI «Institute of Children and Adolescents Health Care of the NAMS of Ukraine». All patients were divided into three groups: 1st group (18 boys) included subjects with the delayed growth and puberty (DG+DP); 2nd group consisted of 10 boys obesity and delayed puberty (obesity + DP); 3rd group involved 9 boys with the delayed puberty. The clinical and laboratory studies included anthropometry, BMI definition, definition of genitals development and pubic hair development in according to Marshall and Tanner and masculinization index (MI), evaluation of structural and functional state of a liver by ultrasound, determination of biochemical parameters (levels of transaminases, alkaline phosphatase, albumin, total bilirubin and its fractions), indicators of the lipid metabolism (triglycerides, total cholesterol, high and low density lipoprotein cholesterol, atherogenicity). Results. The peculiarities of hepatobiliary pathology in boys with delayed puberty have been revealed on the basis of complex examination (clinical, biochemical, instrumental). The presence of signs of biliary dysfunctional disorders tract has been determined in all boys with hypoandrogenism, and in 41 % of the patients the signs of steatohepatosis have been noticed at ultrasound scan. The lipid metabolism changes aggravated with the increase of the degree of delayed sexual development due to the growth of proatherogenic lipid fractions and the reduction of anti-atherogenic lipid fractions, which may indicate an effect of the degree of hypoandrogenism on lipid metabolism. Conclusions. The results of the study indicate the necessity of inclusion of the lipid metabolism parameters and ultrasound scan of the liver and bile ducts into the diagnostic algorithm in youths with hypoandrogenism, as well as the development of protocols for the prevention and progression of hepatobiliary pathology.
